ASB | Clubs & Activities

If you think you should qualify for a free ASB, apply online or use the paper form attached below! Remember that we also must have the consent to share form completed in addition to the application. If you qualify but do not have the consent to share submitted, then it will not be reflected in our system.

The online application should take about 15-30min to complete. The review of your application should be done within 10 days of submitting it. Applying online speeds up the review time!

Please ensure that all fields in each form are filled out; incomplete forms will be returned for completion. Once all forms have been filled out and signed by the coordinating staff member, please place them in my mailbox in the office mail room. After your trip is approved and entered for transportation, you will receive an email confirming the approval, along with the next steps and important reminders.

On Campus Activity Form

Please use the following process for facility usage requests. Start by checking the calendar to find an available date for your event. Once you have a date, fill out the online facility request form. After submitting the form, you will receive a confirmation email, and I will be automatically notified. Once the request is processed, the designated advisor will receive a confirmation that the facility has been scheduled.

This calendar will show you all the current scheduled events with locations and times for your reference when scheduling your event! Select Cascade High for location, then select the filter button to only view facility usage for CHS.

Use this link to submit a request to use facilities at Cascade High School. Make sure to complete all fields and be detailed if any custodial support/ equipment is needed.

*If you are submitting paperwork late and need your abstract approved sooner than the standard two-week processing time, you may visit the DOL website and pay $15 (this fee will NOT be reimbursed) to obtain your record immediately. You can then submit this in place of the Driving Abstract Form along with any other required documents.
